registration form consists of three pages:
1) Registration Guidelines & Instructions
2) Convention Registration Form
3) Competition Registration Form
4) Entry Summary & Fee Calculation.
When sending in your registration, both the 'Convention/Competition Registration
Form' and the 'Entry Summary & Fee Calculation' forms must
be completed and sent in together. It is not necessary to send
the 'Registration Guidelines & Instructions' page.
